If 2 ghasts spawned, but the player went into a nether portal and re-emerged a thousand blocks away, new ghasts could still spawn because the other two are no longer in loaded chunks. If there is 1 ghast in loaded chunks, an attempt to spawn a new one may succeed. If there are 2 ghasts currently in loaded chunks, any attempt to spawn a new ghast will fail.
